Continuing Health Care
Generally, NHS funded care is for people with long-term complex health needs, and because of this they are entitled to free health and social care arranged and funded solely by their local NHS. This is an adult funded service and care can be provided within a care home or your own home.
A healthcare Checklist tool is usually conducted by a nurse or social worker that has been trained to use it. Sometimes the initial assessment tool is all that is needed to receive support with care, however should there be greater needs, then a full assessment will be conducted whereby relevant professionals will be asked for evidence in relation to need. This information is then used to complete a Decision Support Tool (DST). Families and the person in need are able to be involved in the DST assessment. Findings are then put to the ICB, and generally any decisions are put in writing.
Any package of care will be reviewed initially after 3 months, and then generally annually. As needs change, the care package will adapt to reflect this. Decisions can be challenged through an appeal process.
