Council Funded

Generally the person needing care will be in a situation whereby social services has needed to become involved, and you will have been “allocated” a health and/or social worker at some stage. As an example if you are exiting hospital and still require support but can be moved back home. The health/social worker will have conducted a Carers Led Assessment to understand the need before they refer to a partner care organisation.

At Carers Trust Mid Yorkshire we have agreements with Kirklees, Leeds, Wakefield and Calderdale. Within Kirklees we have the ability through our Emergency Service should immediate and short term support be required, or we receive details via the social worker and then contact is made with the carer. Within the other three regions we operate, referrals come through a “framework” whereby care organisations advise if we can assist the person requiring the support. Care agencies have service agreements with councils to facilitate being able to support carers that have been referred.

Generally, a set number of hours is provided, and often each care organisation has a maximum they can provide to each client through this funded service. However, additional hours can be requested and paid for personally. At annual reviews, any changes have to be reported back to the council their appointed agent.

It is worth noting that, referrals from councils do also come through their partnerships with the local health authority, but it is the council that manages the care package that is funded. To use Carers Trust Mid Yorkshire, the person being referred must not have another funded package of care eg direct payments, or activity centre.
